Required Documents

In addition to the registration form, the following documents are REQUIRED to complete enrollment at Thea Bowman Preparatory Academy:

• All academic records (including report cards, standardized test scores, and/or an IEP) from your child’s previous school. 
• A completed DC Child Health Certificate and a DC Oral Health Assessment Form .
• As a resident of the District of Columbia, you must submit the accepted proof of residency as required by law and listed in the Residency Verification Guidelines.
